Ursamin is a SaaS based care coordination platform that helps patients living with multiple chronic conditions get better care. 66M people in the US today are living with MCC, and on average trying to juggle 12 doctors. They have the worst outcomes and the highest costs to our healthcare system, due to the US focus on disease state, vs. whole person health. The healthcare community, while trying to help, is woefully understaffed, and struggling with heavy burnout numbers combined with a shortage of new professionals entering the health industry. The healthcare \"crisis\" is real - and needs thoughtful solutions that will help guide us out of a downward trajectory.Our mission has been and will continue to be to help as many patients as possible by: automating low level processes, rebalance the workload and empower patients to help drive their own care, including the family caregivers, and helping the providers to work together as one team for MCC patients. By creating a collaborative environment for the users, enabling the team to work together around one shared care plan that travels with the patient and empowering them to intervene or adjust the care plan as the patient moves along their health journey.
