Provide social work support to patients’ who suffer from End Stage Renal Disease (ESRD) and are utilizing Home Hemodialysis and Peritoneal Dialysis modality options-Conduct biopsychosocial assessments, assess needs and coordinate resources as appropriate-Provide education about dialysis modality treatment options, kidney transplant, among other dialysis related topics (i.e. access to medication, dietary/fluid restriction, coping, etc.)-Coordinate psychosocial referrals for: food, shelter, financial assistance, housing, legal services, etc. Referrals are based on patient need-Collaborate with interdisciplinary team to ensure all patients’ needs are addressed and appropriate resources are provided-Participate in monthly clinical improvement meetings with interdisciplinary team to discuss clinical outcomes and ways to strengthen continuity of care.

My undergraduate studies were conducted at Saint Mary\'s College of California where I received my B.A. in Sociology with a minor in Ethnic Studies. I also studied at the University of Southern California School of Social Work and received my Master\'s of Social Work (MSW) with a concentration in health. My passion for social justice and working toward eradicating social injustice, motivates me to seek employment that strives to address social issues, on the micro, mezzo, and macro levels. Specifically in the area of health, health education and prevention.
