MedZed provides community-based services to address the Health-Related Social Needs (HRSNs) of Medicaid and dual eligible members who have become disengaged from primary healthcare. The company’s scalable field-based teams of Community Health Navigators (CHNs) find, engage and work one-on-one with members to understand and address their complex social and health-related needs, connect them to resources and guide them through the nexus of community organizations, providers and health plans. By partnering with Medicaid and Medicare Advantage health plans, MedZed simultaneously addresses the essential human needs of those at-risk individuals who account for 30-50% of overall medical expenditures, while tackling the business challenges of reducing inappropriate utilization and total cost of care. Over the past 11 years we have cared for over high-risk, hard-to-reach people with complex social and health-related needs in 11 states and completed almost 1 million home visits and member calls. Caring for members for an average of nine months, we are approaching hours of patient care. The results speak to the impact we\'ve achieved - 40% engagement rates, 38% decreased ED utilization, 54% decrease in inpatient admissions and 54% decrease in total medical costs.
