The Spiras Health care delivery model includes a combination of home-based services, telehealth, two-way digital communications and remote patient monitoring. By identifying and assessing members with an elevated probability of avoidable outcomes and costs, we are able to develop actionable plans of care and address barriers. Leveraging SpirasConnect, we optimize engagement and activation always focusing on building trusting relationships. Spiras Health targets patients early in their disease lifecycle and can manage them through palliative care, or until a hospice transfer is appropriate.
