Implemented a standardized pharmacist-based anticoagulation monitoring service at an established state-wide outpatient cardiologist practice in 2021 with an interdisciplinary team of pharmacists and medical assistants to build relationships with outpatient and inpatient providers and clinical support staff. Monitor and interpret laboratory results and adjust medication regimens per existing protocol and collaborative practice agreements in accordance with established policies and procedures and federal and state rules and regulations.**Key accomplishments: Step up as point person for peri-operative management of anticoagulation, medication access concerns, symptom calls within scope of practice, etc. Responsibilities include pre-procedure blood thinner interruption and bridging recommendations to cardiology team utilizing risk-benefit expertise per patient scenario and assistance with clerical workflow for clearance documentation for providers and secretarial support staff. Precept and creates curriculum for PharmD candidates for IPPE and APPE pharmacy school rotations; created material for lectures specific to anticoagulation for healthcare professional candidates. Participate in training of pharmacist colleagues and clinical support staff. Foster community, resilience, and self-care amongst pharmacy colleagues by creating a Pharmacy Wellness Retreat practice across affiliates.

I am privileged to have my Doctorate in Pharmacy from the University of Rhode Island (Class of 2019). I am currently working for the Brown University Health Cardiovascular Institute as an anticoagulation ambulatory care pharmacist. I have worked in a range of practices including retail pharmacy, long-term care (community mental health), hospital pharmacy (centralized and decentralized to transition of care, critical care, and internal medicine), and ambulatory care cardiology clinics. My passions in pharmacy and healthcare include transitions of care, medication therapy management, community outreach, medication access, and addressing health disparities and gaps in care. I care deeply for the community around me and have experience with educating others and advocacy. I take pride in serving as a mentor to others and precepting pharmacist candidates and residents. I am an individual who addresses new ideas and tasks by looking at the big picture, and constantly strive to be a leader and nurture my professional relationships and my work.
