Develops person-centered treatment plans with individuals to support them towards their recovery goals, checking in on progress over time, modifying goals as needed. Assesses clients to determine appropriate level of care, and coordinate additional services as needed. Complies with privacy guidelines to protect client information. Attends team meetings to discuss ongoing client care and coordinate team needs.
