Responsible for safely managing complex and chronic cases, discharge planning and coordinating the transitional outpatient recovery process on the fast paced 54-bed orthopedic trauma unit• Assess patient functional status, home environment, support system and individual needs• Review all multidisciplinary electronic medical records, progress notes, discharge criteria, complete nursing plan of care• Collaborate with social workers, doctors, therapists, chaplains, and case managers to implement safe transition home • Develop and implement patient-specific discharge and outpatient follow up plan to promote health and optimum recovery • Evaluate medical necessity and physician order for home health, work with case managers and social workers to document home health order, coordinate delivery. Provide referrals, FMLA paperwork, and supplemental resources as needed • Assist PT/OT in family training for safe use of orthopedic equipment • Educate patients on injury-specific restrictions, precautions, weight bearing status, proper body alignment and mechanics to prevent further injury. Perform discharge education teaching for patients and family • Advocate for patient safety, identify and collaborate resolution of any possible barriers/issues in follow up home care/return to work
