Program Director for Money Follows the Person @State of Vermont
The Money Follows the Person (MFP) program supports state efforts for rebalancing the long-term services and supports system so that individuals have a choice of where they live and receive services. As Program Director, I manage a dynamic team of four in supporting Vermont\'s participants in the program, guiding outreach and educational efforts to teach hospitals, skilled nursing facilities and case management agencies about the program, as well as directing our 10+ contractors doing work in their communities to ensure folks have the resources they need to thrive at home. MFP is a dynamic program and I am thrilled to steward this program into the future.
