Responsible to undertake a holistic assessment of the ongoing needs of the client including their health, social, cultural, rehabilitation, educational, vocational and emotional needs- Formulating a comprehensive case management plan and develop goals in liaison with the client and all involved and monitor the agreed goals on a regular basis- Facilitating and co-ordinating the services delivered to meet the identified needs of the client using a multi-agency approach- Recruiting, co-ordinating and managing a care team, providing training and supervision as required- Developing effective collaborative and co-operative working relationships with the client’s support system, including family, carers and other professionals- Offering advice, support, information and advocacy to meet the needs of the client and their significant others- Responsible for undertaking detailed risk assessment and develop appropriate risk management plans as required- Maintaining clear and effective written records of all Case Management activity and interventions made with, or on behalf of, the client- Maintaining a high standard of knowledge, skills, conduct and to maintain knowledge of all relevant legislation including employment law and health and safety for support workers.
