About Sangeeta Kopardekar
Sangeeta kopardekar is the founder of the Department of Geriatrics at PAMF. The Geriatrics department started with 2 geriatricians and the model has successfully grown to 13 geriatricians at PAMF. In addition, also established an outpatient palliative care program at the Camino region in PAMF. The geriatrics and outpatient palliative care programs care for elderly patients and those with serious illness at home, in nursing home and in the clinic setting with the support of an interdisciplinary team. Served as Medical Director of Bundled Payments at El Camino Hospital and facilitated establishment of hospital palliative care service and case management program. Served as Geographic Medical Director for primary care, Geriatrics, Urgent Care, Psychiatry and behavioral health. As Medical Director for Advanced Illness Management(AIM), worked on merging outpatient palliative care and AIM. Current interests are population management and providing high quality care across the Care Continuum.
