Inpatient & outpatient continuity of care position working with patients and families affected by rare hematological malignancies, whose treatment plans often include bone marrow transplants. My role includes a strong end of life component where a social work intervention is essential to support both patient and their families. Responsibilities includes an in-depth psychosocial assessment as part of a patient\'s evaluation for transplant, psycho-education on the profound lifestyle changes transplants often bring, mental health counseling, and safe discharge planning. This role is an integral constituent of a highly skilled multidisciplinary team of medical providers and case managers. In addition to providing community referrals, I run a post-allogeneic stem cell transplant support group for survivors.
