   Developing and implementing Care Coordination Program processes and workflows in partnership with clinical staff-Establishing and maintaining relationships with providers and case managers at local hospitals, clinics, and community organizations to increase the reach of HEP’s Prevention and Outreach and Medical Case Management programs-Conducting hepatitis and infectious disease testing via venipuncture and point of care tests on-site as well as at multiple community outreach sites throughout the Seattle Area-Providing comprehensive medical case management, including health education, health systems navigation, and treatment navigation for substance use for clients enrolled in medical case management-Working with clients and their treating providers to enroll clients in hepatitis C treatment as well as treatment for other co-occurring infectious diseases, including HIV, and Hepatitis B-Scheduling clients’ medical appointments throughout their treatment process, providing reminders and transportation assistance, and ensuring clients complete necessary follow-up testing and appointments post-treatment-Utilizing HEP’s client database to document detailed case notes, interpret and enter lab results and upload medical records, and regularly pull reports while adhering to strict data security and confidentiality protocols-Enrolled participants in UW Medicine hepatitis C research study, in partnership with Kelley-Ross Pharmacy, collected client data, and coordinated follow-up appointment for clients-Co-authored UW hepatitis C treatment research paper, reviewed and provided feedback on manuscripts-Providing phlebotomy training and education regarding trauma-informed testing practices to colleagues at HEP and partner organizations-Enrolling clients interested in MOUD through HEP’s on-site buprenorphine clinic-Conducting outreach to provide harm reduction supplies and essentials at homeless encampments-Staffing and stocking HEP’s syringe service program
