Seasoned director, manager; global industry analyst and competitive market researcher; skilled in benchmark design and provider partnership modelsCurrently applying my operations and industry research background to healthcare. My focus at UMass Memorial Health is building the foundational elements that drive the expansion of the UMMACO (Accountable Care Organization), and establishing scalable application and onboarding processes. I am involved in overseeing the evolution of the Preferred Post Acute Care Network in support of value-based programs (e.g. ACO, Bundled Payment Care Initiative-Advanced, Primary Care First, etc.) This encompasses collaboration with skilled nursing facilities, home health agencies, inpatient rehab facilities, long term acute care hospitals and hospice agencies. The focus extends from partnership development, to benchmark modeling to performance monitoring and analytics, in line with the changing regulatory climate and its impact across different post-acute partner types. My cross industry knowledge and managerial experience are valuable assets in my healthcare roles. I pull from an array of experiences and best practices. These extend from building benchmarks in software testing services to leading operations teams at multiple telecom companies (iBasis, NaviPath, GTE Internetworking). My honed skills in quality assurance and sophisticated data trending are especially effective in strategic and tactical planning. I have driven multiple international projects through all phases of development and deployment. Many of these have involved driving down cost structures while building expansive networks and restructuring processes.As an adjunct to my organizational and technical endeavors, my studies in expressive therapies support the work of my clinical team mates who are focused on delivering high quality patient centered care to our high risk ACO population and behavioral health programs.    Oversee care navigation and post-acute care management in supporting the UMass Memorial Medicare ACO of ~ beneficiaries. Collaborate with UMMH inpatient case management and social work leadership across our member hospitals to help patients transition to next appropriate level of care, whether it is to a skilled nursing facility (SNF), inpatient rehab facility (IRF) or home with home health or hospice services. Facilitate leadership discussions with preferred SNF, home health and hospice agencies and partner with facilities outside of the network to help drive improved quality of care for our mutual patients. Supporting deployment of a central triage behavioral health workflow to enable providers and their patients to obtain easier access to behavioral health services within the UMMH network, as well as in coordination with our internal and external community behavioral health partners.Involved in supporting new models of care in the home, such as our recent randomized control trial in enabling a Subacute Rehab at Home grant. Collaborate with our Mobile Integrated Health and Remote Patient Monitoring teams. Have helped establish a payor agnostic referral triage workflow and team so that providers and their patients can receive guidance in navigating to care management teams associated with commercial payors, Senior Care Options (SCOs), other Medicare and Medicaid ACOs, etc.
