Bilingual Rapid Entry Linkage Coordinator @Positive Impact Health Centers

Use independent decision-making to develop and maintain training and reference materials, presentations and job aids using department tools, templates and standard operating procedures• Coordinates enrollment in PIHC\'s Rapid Entry Clinic for the following patients: newly diagnosed with HIV, living with HIV but never in care, and/or those without medication for more than six months• Provides education to patients about the importance of HIV medical care and medication adherence for persons living with HIV• Screens patients to determine the need for non-medical services, including mental health, medical case management, and substance abuse services, utilizing the Atlanta EMA screening tool and the Acuity Scale• Schedules medical appointments (labs, clinic visits) for patients with the Rapid Entry clinic providers, utilizing the Electronic Medical Record (EMR) system• Enters patient information and documentation into Cerner PowerChart and Specialty PM• Coordinates with the clinical team to ensure new patient laboratory tests are collected• Checks Ryan White eligibility by collecting eligibility documentation• Develops a sustainable HIV care plan with each client that focuses on mitigating barriers to care, determining benefits, and linking to a long-term care provider
