Mindful Growth Foundation (MGF) is a licensed Short-Term Residential Therapeutic Program (STRTP) serving youth impacted by trauma, system involvement, and complex behavioral health needs.MGF partners with families, county agencies, and interdisciplinary teams to deliver trauma-informed, culturally responsive care rooted in the Integrated Core Practice Model (ICPM) and the Child and Family Team (CFT) process.Serve as the primary contact for the child and family, ensuring alignment with the ICPM and CFT model.Lead, facilitate, and document Child and Family Team (CFT) meetings.Develop strengths-based, youth- and family-driven treatment plans.Coordinate communication between caregivers, natural supports, MGF clinical staff, county partners, schools, and community providers.(DMH, DCFS, LAUSD, etc)Support families in navigating community systems and accessing needed resources.Monitor progress toward treatment goals and update plans as appropriate.
