As a Family Support Coordinator. Case Manager, and CPRS S. My responsibilities included monitoring and assisting Peers onsite during program outreaches. Manage a caseload of 60+ participants, and connect with each once a week, bi-weekly, or monthly. Host in-person Spanish and English groups monthly. Building coping skills and various activities for participants. Meet with participants daily in community settings, for a program project, “Walk with the Participants”. Evaluate families, document their daily progress, and goals, and develop plans for their home and community life. Help families obtain support in the community through available resources that are needed and available. Help to create plans for families to pursue their goals. Plan and organize community outreaches. Create progress and service surveys. Monitoring my program’s 24-hour hotline for participants in need of treatment
