Embedded in a Primary Care Clinic with Attending and Resident MDs, Nursing, Social Work, Mental Health, Pharmacy and Dietician- Immediate or same-day clinic access for urgent, acute or sub-acute needs. Providing screenings and more specialized referrals if needed- Physiotherapy and functionally-focused evaluations and therapeutic exercise (ideally available within hours as opposed to days or weeks in traditional access models)- Multi- and Inter-Discipline collaboration with a variety of other care providers within the PACT (Patient-Aligned Care Team)- Providing education (formal and informal / curbside) to MD and Nursing staff on the roles and responsibilities of Primary Care PT and Physiotherapy in general- Maintains lines of clear and open door communication with the team on specific patients to assure that their needs are addressed in the most efficient and timely manner.
