Advocacy and empowerment of patients informed by assessments of the social determinants of health (SDOH). After initial appointment, contacted and created bonds with patients to assist them in creating a customized solution to overcome any barriers that were identified during their SDOH assessment. Trained to identify existing community resources to address the needs of patients.The Mobile Outreach Clinic provides primary medical care services at no cost to the patient and will provide services to all individuals regardless of their ability to pay or because of their race, color, sex, national origin, disability, religion, or sexual orientation.
