QIC) Lead in Quality and Performance Improvement for SeaTac Clinic. Responsible for continuous process improvement. Engage front office, medical assistants, RNs, behavioral health, pharmacy, customer service reps, call centers, and leadership to develop workflows and process improvements.• Monitor and report out on all outcomes data such as Medication Adherence, A1c control, HTN, Depression, Well Child visits and Combo 10. Distribute to providers and staff with recommendations. Work with the Care Team on PDSAs for assigned measures.• Facilitate and lead monthly staff Care Team meeting to discuss where we are with all measures, demonstrate where we are succeeding, and where we have opportunities to improve. Engage the staff in sharing ideas on small tests of change.• Organize, process, track, and coordinate patient care to ensure completion of services such as routine and high-risk mammograms, cancer treatment, and services from the referral to the completion. Ensure correct and appropriate documentation is uploaded into the EMR. • Monitor and flag patients for high risk follow up. Review medical records from external organizations to ensure patients were able to schedule with specialist. Assist the patient when needed. Follow the patient through the process to completion of treatment. Ensure providers receive reports and recommendations. • BCCHP (Breast, Colon, Cervical Health). BCCHP is a King County service for patients that qualify for free cancer screening. Review patient information to identify patients that qualify for this program. Assist with the application paperwork for coverage and submit to King County. • Manage monthly mammogram services in clinic. Work with external Mammography Van to schedule and manage appointments. Ensure appropriate paperwork is provided and patient rooming runs smoothly.
