The objective in the field of neurodiagnostics is to have this field recognized as a professional state regulated field, the same as Respiratory Therapists, RNs and MDs, which requires state licensing. There are to many independent IONM companies not monitoring under the ASET guidelines, no peripheral responses such as Erb\'s point (brachial plexus) which lets the surgeon know that the position of the patient is adequate, no popliteal fossa potential which can assess the peripheral system from the ankle to the knee and/or localize an abnormaility in the CNS by conduction times. I have witnessed speakers off for free running EMG. I have also documented asymmetrical stimulation intensities just so the popliteals can match in amplitudes. I have witnessed all of this in independent IONM companies. It is not possible to report this type of monitoring or educate the tech and other personel without state licensure. No CNIM has the right to call themselves ‘neurophysiologist’ without an MD or PhD behind their name or monitoring neurophysiologist. No IOM company or hospital should be advertising for a ‘surgical monitoring neurophysiologist’ based on having a CNIM as these individuals do not have the proper education to document that they are actually ‘neurophysiologists’ and ASET & ABRET should have made an official statement against this by now and have not officially done this. I believe Neuromonitorist is acceptable. Anyone with a REEGT, REPT, R. NCT, or CNIM can get into the IOM business with or without a degree without any state regulations and call themselves a neurophysiologist if they have ABRET credentials. One is not to call themselves a neurophysiologist unless that person has attended a program in a university program for it and usually these people that are true neurophysiologists are MDs first.
