Patient Flow Expeditor | Throughput Optimization & Discharge CoordinationPartnering with Case Management, Operations, and interdisciplinary teams to lead the development and implementation of the Patient Flow Expeditor program. This role is designed to improve patient throughput by proactively identifying, addressing, and resolving barriers to care progression and discharge across departments and throughout the healthcare system.Key responsibilities include-Coordinating real-time communication between clinical and non-clinical teams to facilitate timely patient transitions-Monitoring patient flow from admission through discharge, ensuring delays are minimized and care progression remains on track-Working interdepartmentally to troubleshoot and eliminate discharge barriers, including delays related to diagnostics, consults, post-acute placement, transportation, and documentation-Utilizing system-wide resources and data to anticipate bottlenecks and implement solutions that enhance operational efficiency and patient outcomes-Serving as a central point of coordination to streamline workflows, align priorities, and drive collaboration across nursing, providers, ancillary services, and support departments.This role plays a critical part in improving hospital capacity, reducing length of stay, and ensuring a seamless patient experience from entry to exit within the healthcare system.
