   I counsel patients biweekly to create and help them navigate an individualized nutrition plan to lead them to achieve their personal health goals and empower them to succeed long-term. I collaborate daily with Form Health® Board Certified Doctors to identify patients\' biological blockers to weight loss and develop a customized weight loss path that includes nutrition, physical activity, mindset shift, and FDA-approved medication if appropriate. As liaison between the Registered Dietitian team and Product & Development team, I act as point person for Form Health’s app designer and patient experience team in order to provide clinical feedback and app improvement ideas to enhance functionality and user experience, and to increase efficiency of the clinical team.
