Completes assessments using the KEMPE family stress checklist on prenatal parents and parents with children less than two weeks of age to identify high risk parents.Makes appropriate referrals to include, but not be limited to, Healthy Start, WIC, Medicaid, Parenting Classes. Introduces and explains the Healthy Start program, encouraging parental participation.Completes and enters assessment materials and other narrative documentation in the computer system according to Healthy Families America policies and procedures.Provides limited counseling services regarding personal adjustments, family relationships, and/or other situational problems. Utilizes interview techniques, problem-solving skills, and knowledge of community resources to assist in the resolution of the family\'s problems. As required, may utilize crisis intervention and short-term counseling and/or referral.Establishes and maintains manual and computerized records and files according to Healthy Families America protocol. Prepares and submits reports in a timely manner. Attends regular staff meeting and in-service training.
