DUTIES INCLUDE: As a Case Manager, my duties include utilization reviews of the patient’s charts daily for both Medicare patients and insurance companies to ascertain if the patient meets criteria to be in the hospital. My duties also included risk assessments for proper Discharge Planning and Disease Management. I am also a dedicated Discharge Coordinator that focuses on the quality of care for the patient while in the hospital and post discharge. I determine the best and safest DC plan. Additionally, I play a very important role in decreasing the length of stay of patients that no longer meet criteria per CMS guidelines thus increasing revenue for the hospital. I work very closely with Physicians and nurses at our facility to improve the quality of patient care, the physician’s HCAP score/patient outcomes, and decrease re-admissions thus improving the revenue for the hospital. We work as a team.
