   We are a digital health equity startup passionate about improving the quality, consistency, and equity of health outcomes for underserved patients and their caregivers. We\'re creating the gold standard for patient experience and access to care through our digital coordinated care hub. Our care platform elevates the patient\'s clinical/behavioral/social well-being by: 1) Aggregating disparate clinical, behavioral, and social data, and closing gaps in care 2) Connecting patients with social and educational resources to reduce burden of illness 3) Applying AI-based analytics to optimize healthcare utilization As Co-Founder and CTO, I\'m responsible for overseeing all of aspects of product management / design / development, software engineering, devops, data science and analytics, cloudops, IT infrastructure, and security compliance.
