About Raye Serratos
I am a Psychiatric Nurse Practitioner, graduated with my MSN with focus on PMHNP August 2024. Passed Wisconsin state APNP boards in November 2024. I am excited to continue my journey with Ascension as a PMHNP because I value the mission and vision, including integrity, wisdom, and dedication this team has to improve the individual mental health outcomes of the patients and the community and the education of upcoming practitioners. I look forward to providing improved outcomes for my patients, bettering the community, and the opportunity to educate future practitioners. My patient care goals are to enhance functioning in school, work, and the everyday life of my patients. Building a support system of people, including myself, and offering tools, like, but not limited to, utilizing medication management, therapy, prioritizing sleep and nourishment to handle life\'s stressors better. Providing care with an individualized approach and collaborating with patients, their caregivers, and other care team members for optimal patient outcomes. I will ensure strong lines of communication between the patient, the patient\'s caregivers, and the care team to achieve optimal patient outcomes together.
