Our mission is to connect exceptional pharmacists with patients who are at a high risk of experiencing medication-related problems. Our nationwide network of consultant pharmacists provides quality comprehensive medication reviews and targeted medication reviews telephonically, face to face, and in clinical settings to patients and/or their caregivers as a means to improve medication outcomes. We work with Medicare Part D Health Plans, state-funded Medicaid plans, private insurers, employer groups, and other MTM providers. It is our vision to provide the highest quality and quantity of MTM and CPS interventions in the industry based on our 5 core values of Connection, Health, Autonomy, Integrity, and Mastery.
