Provide advanced clinical pharmacy services for specialty patient populations by optimizing medication therapy through drug selection, dosing, titration, monitoring, and discontinuation based on patient-specific clinical parameters.• Manage complex pharmacotherapy cases in collaboration with physicians, advanced practice providers, and multidisciplinary teams to improve therapeutic outcomes and reduce medication-related complications.• Lead medication therapy management for high-risk medications and disease states throughprotocol-driven care, collaborative practice agreements, and evidence-based clinical decision-making.• Perform comprehensive medication reconciliation at admission, transfer, and discharge to improve medication accuracy, supportc are transitions, and reduce readmission risk. • Monitor and interpret laboratory values, pharmacokinetic data, and clinical response to guide therapy adjustments and ensure medication safety and efficacy. • Provide real-time drug information and clinical recommendations regarding adverse effects, drug interactions, contraindications, and guideline-directed therapy. • Support formulary management and drug utilization initiatives through evaluation of targeted medication classes,costeffectiveness analysis, and utilization reporting.• Develop and implement clinical protocols, standing orders, and treatment pathways to standardize care and support quality improvement initiatives.• Educate patients and caregivers on medication adherence, administration techniques, side effect management, and disease state management.• Deliver education and in-services for pharmacists, physicians, nurses, and students on specialty therapies, new medications, and clinical guideline updates. • Ensure compliance with regulatory and accreditation standards including DEA.• Participate in quality, safety, and process improvement initiatives to optimize medication-use systems and reduce preventable adverse drug events.
