I am a senior healthcare executive and advanced practice clinician with over 20 years of experience leading population health, care management and chronic disease programs for adults with physical disabilities and high-acuity needs.My primary work focuses on designing and operating patient-centered population health strategies that improve access, outcomes, experience, and sustainability within Medicaid and managed care environments. My approach is grounded in clinical care delivery, collaboration with providers and plans, and partnership with patients and families—together we can make a difference. I have the honor to serve as Senior Vice President of Population Health at Independence Care System, where I lead organization-wide population health strategy, quality improvement, and care model innovation. In partnership with my team, we have developed New York State’s first Health Home program dedicated to adults with physical disabilities where we reduced avoidable hospitalizations and embedding the patient voice into program design and staff training across the organization.I maintain active clinical practice as a Nurse Practitioner in a multiple sclerosis center, with the goal of ensuring that organizational decision-making remains grounded in real-world patient experience. I am nationally engaged in professional and clinical leadership through long-standing service with the National Multiple Sclerosis Society and the International Organization of Multiple Sclerosis Nurses.I am driven by the belief that sustainable population health is possible and is dependent on investing in both patients and the workforce that serves them.
