Responsibilities:Responsible for overseeing provision and allocation of high-quality personalized care to all our service users; care intervention in line with clients’ care plan, needs and SLAsconnected to quality care delivery.• Leading the allocation and coordination of branch staff by managing theallocation and modification of care shifts for all staff within the Branch, on a rota basis as well as emergency cases when required, to ensure continuous delivery of high-quality services to users.• Liaising and collaborating with partners and other professionals to provide highest quality service to all our clients.• Ensuring that patients with complex health needs receive timely and coordinated interventions across various healthcare services.• Identifying and addressing gaps in patient care, making necessary adjustments to care plans to ensure continuity of care.• Providing client care call cover in cases where designated staff became unavailable• Documenting all care coordination activities, including assessments, interventions, and communications, while also providing clear and concise reports on patient care outcomes, identifying trends and opportunities for improvement.• Responding promptly to urgent care needs or critical changes in patient conditions, coordinating appropriate interventions.• Managing escalation protocols effectively to ensure high-risk patients receive immediate attention.• Promoting teamwork, ensuring the support team work well together to encourage and boost team collaboration and team spirit. • Managing and resolving conflicts within the branch staff to ensure team spirit• Maintaining accurate and up-to-date patient records in line with data protection regulations.
