Manage a caseload of 6 on the adolescent unit and 10 on the adult unit, patients with substance use disorder and co-occurring disorder. • Complete biopsychosocial assessments and assess for mental and substance use disorders. • Develop comprehensive treatment plans specific to each patient and conduct reviews.• Provide clinical recommendations and progress notes for insurance reviews. • Complete weekly individual and family sessions on both the adult and adolescent unit and couples sessions on the adult unit. • Provide case management and discharge planning support including recommendations for step-down and aftercare.• Facilitate group sessions on ACT skills, SUD education, relapse prevention skills, and healthy relationships. • Facilitate daily process groups with caseload. • Utilize crisis intervention and de-escalation skills to prevent unplanned discharges.• Conduct suicide assessments and re-screens and refer to higher level of care in cases of high suicide risk.• Participate in weekly multidisciplinary treatment team meetings presenting case conceptualization and progress reports for each patient on caseload. • Document all sessions, contact notes, interventions, and consents in an EMR.
