In my role as a Population Health Clinical Pharmacist at Banner Health, I have taken the lead in optimizing medication therapy management for diverse patient populations. My focus is on enhancing clinical outcomes while also working to reduce overall healthcare costs.Key achievements in this role include:• Spearheading the design, execution, and optimization of clinical programs within the Insurance Division, leading to improved quality and cost-effectiveness of pharmaceutical care. This effort involved coordinating the review and processing of an impressive 3,700 Comprehensive Medication Therapy Management claims.• Delivering impactful executive-level presentations to clients and stakeholders, clearly articulating program impacts and strategic initiatives. My collaboration with both internal and external partners has enhanced treatment access and bolstered patient support significantly.• Innovating the medication outreach process and refining call script procedures to boost patient engagement. This included providing ongoing training and support to case managers and healthcare providers, ensuring optimized service delivery across Arizona.• Serving as a Subject Matter Expert (SME) for clinical services and products, where I successfully negotiated performance guarantees for the new Pharmacy Benefit Manager (PBM) implementation with Express Scripts in 2022.• Developing a robust SBAR framework to secure increased resources for expanding in-house pharmacy services. This initiative reduced our reliance on outside vendors while enhancing interoperability and improving efficiencies, ultimately leading to better health outcomes.• Authored and published over a dozen medical articles and blogs, providing expert guidance on medications for both patient and healthcare provider education.
