I serve as the senior hemostasis-coagulation expert for the entire Northwestern Medicine system. In addition to the usual high-volume testing, I am responsible for the NMH Specialized Hemostasis Laboratory which performs many special hemostasis tests including Factor inhibitor (Bethesda) assays; platelet function and antiphospholipid antibody assays.Some of my contributions and responsibilities include:*Provide medical direction for the development and implementation of all new tests in the hemostasis laboratory including a quantitative fibrinogen assay, immunoturbidometric Factor assay, platelet function tests, thrombophilia workups and anticoagulant monitoring assays including for new oral anticoagulants rivaroxaban and apixaban.*Implementation of viscoelastic coagulation assays for surgery, obstetrics and trauma patients, which will improve recognition and treatment of thrombocytopenia, hypofibrinogenemia and fibrinolysis in these settings.* Implementation of a new fast turn-around time immunoturbidometric assay for heparin-induced thrombocytopenia (HIT) which will facilitate rapid diagnosis and treatment of patients suspected of having HIT.* Provide medical and laboratory expertise as a member of a NMHC special committee to monitor heparin therapy, for implementation of the anti-Xa assay for heparin monitoring, which will provide more accurate and reliable therapeutic heparin levels than the conventional use of the aPTT.* Provide medical direction for quality initiatives in safe transfusion practices and appropriate laboratory test utilization and interpretation.
