Provide comprehensive patient-oriented clinical services on the inpatient general medicine and neurology/stroke wards - pharmacotherapy monitoring, interdisciplinary participation, drug use evaluation, adverse drug reaction reporting, medication counselling, medication incident reporting and follow-up, and medication history and reconciliation services • Project Co-Lead “One vial, One Patient” – transition from multi-dose to single-use vial practices across Operating Room, Endoscopy, Labour & Delivery, and Minor Procedures Clinic; ensure compliance with Accreditation Canada Required Organizational Practices (ROP), comprehensive cost-benefit analysis (alternate products, external procurement or CIVA preparation), and implementation strategy.• Pharmacist lead for Evidence-2-Practice Sickle Cell Disease Working Group - developing advanced care plans to provide optimal pain control for this population• Pharmacist representative for Sickle Cell Advisory Group – aim to improve quality and consistency of care in this population• Pharmacist representative for NYGH stroke QBP- outcome of reducing length of stay, improve patient education, evidence-based practice for discharge medication list• Developed Naloxone Kit prescribing protocol for inpatients at NYGH – patient safety and risk mitigation• Review order sets for the Computerized Physician Order Entry (CPOE) system• Participate in drug information and other educational activities within the department, on the nursing units and throughout the hospital (e.g Pharmacy Awareness Week, Clinical Sharing Sessions, etc.)• Maintain activities in all drug use and control functions: drug distribution, central IV admixture program (CIVA), computerized medication order processing, formulary system, inventory control system, and drug procurement.
