Initiate telephone calls to all family members and close friends of Hospice patients on the bereavement referral sheet, inform of bereavement services, document interaction, counsel and make appropriate referrals. • Provide short-term individual, family and group bereavement counseling-Parent Loss, Understanding Grief, Finding Balance in Grief with Mindfulness, Next Steps, and documentation • Return community telephone calls, make appropriate referrals, and documentation • Perform telephone assessment of potential group participants. • Register callers for bereavement classes. • Confer in IDT meetings with Social Workers, Nurses, M.D. and Spiritual Care Coordinators • Participate in recruitment, training, support, coordination and recognition of volunteers • Develop and/or participate in education programs and workshops for patients/families, staff and community at large * Mindfulness and Stress Management Presentation to Staff and to Volunteers * Development and facilitation of new grief support groups-Mindfulness and Journaling; Taking Next Steps
