   Grant-funded position, addressing Social Determinants of Health (SDoH). Provide assistance in securing health resources and in-kind support from multiple sources, collaborate with local organizations, and lower health care costs by addressing health concerns early and ensuring continuity and follow-up to health care. Eliminating barriers to health care such as transportation and co-pays through client support funds, and lack of medical coverage through assistance with applications and enrollment processes. If needed, the Health Resource Navigator works directly in the home with a client and their caregiver to develop a service plan to meet the client’s health and disability service needs; thus reaching individuals with mobility concerns, those socially isolated, or without transportation. The Health Resource Navigator is responsible for responding to referrals within 72 hours. The Health Resource Navigator schedules intake meetings and makes subsequent referrals, linkages, service plans and follow up to ensure access to health care. The Health Resource Navigator provides linkages to the client and their support system regarding services available in the community, assists the client in gaining access to services, aids families in obtaining information on available services and resources. When appropriate, the Health Resource Navigator will provide community educational workshops on relevant topics such as basic health needs, accessing health coverage and entitlement options, disability services, and other general health and well being topics.
