I provid early age, elderly and disabled clients with home-based care that included daily assistance for bathing and grooming and dressing and meal preparation to help them maintain their independence and dignity. • I tracked patient safety and health status through vital sign checks and mobility changes and medication alerts which I then reported to families and nursing staff. • I created customized care plans that addressed each client\'s physical requirements and emotional and social needs while developing strong bonds with my clients and their families. • I helped clients with their mobility needs through transfer assistance and the use of gait belts and walkers and wheelchairs to stop accidents and guarantee safe movement. • I documented all care activities through proper documentation of daily progress notes which followed both agency standards and state health regulations.
