Recovery Coordinator @Change Grow Live

Change Grow Live (CGL) is a national health and social care charity, helping individuals with drugs and alcohol challenges and other related issues. My role as a Recovery Coordinator (RC) involves;•Managing wide range of caseload( high medium low) with the main responsibility of providing pharmacological, psycho-social and recovery support to individuals addicted to opioids by using Opioids Substitute Treatment (OST).•Supporting clients from point of entry into the service, through their treatment/recovery journey and up to treatment exit utilising evidence based approaches.•Providing screening, assessment, and recovery planning and onward referral for rehab and detox.•Reducing drug and alcohol related harm to clients and the wider community.•Promoting carer, service user community involvement and ensuring access to partnership services; liaising with probation, housing providers, GP surgery, mental health team etc to achieve person centred care.•Working with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination.•Research, design and prepare subject matter and materials for group’s alongside delivering structured content within group settings to service users at various stages of recovery.
