Conducted a retrospective chart analysis of up to 200 patient records to identify key timeline checkpoints, evaluate medication reconciliation practices, and assess system-level factors affecting timely BPMH completion.Key responsibilities and contributions- Extracted and analyzed patient data including ER triage time, admission and consultation times, and BPMH completion/reconciliation rates- Investigated the impact of BPMH orders, medication volume, and admission timing on reconciliation efficiency- Identified trends in BPMH workflows to support quality improvement initiatives- Contributed to research methodology design and data organization in collaboration with a clinical team- Aimed to support safer transitions of care by informing hospital policy on medication reconciliation timing.
