About Norma Stracener
I\'m often described as a transformational team leader and collaborative director of post-acute care services. I drive change by innovatively building community teams and integrating technology that helps manage daily operations and improves performance efficiency.As a leader, I believe in developing community networks that offer health systems support and open information exchange that informs hospital readmissions management, lowers costs, and improves patient outcomes. I work to connect people at various levels across communities and healthcare systems to ensure that we’re all working to improve population wellness. When confronted with performance gaps, I develop process frameworks that foster workflow change and provide progressive analytics.
