Founded in 2003, SPG provides medical care to patients across the Phoenix Metro area in hospitals, long term acute care facilities, acute rehabilitation facilities, skilled nursing facilities, assisted living facilities/group homes and in private homes. SPG has experienced steady growth thanks to the patient-centered philosophy and our high-quality physicians and partners. At SPG, it is all about what is best for our patients. SPG has wonderful team of Board Certified, Internal Medicine physicians, Clinical Psychologists, Nurse Practitioners, Social Workers, Transitional Care Specialists and our internal support services staff that all demonstrate strong communication skills, compassion, and dedication. At SPG Virtual Care™, we are a team of board-certified medical professionals and medical technicians who have formed a network in order to help those who need services in the comfort of their own location. We are dedicated to problem-solving and managing medical conditions and easing suffering, as well as providing preventative care in order to prevent chronic conditions from developing or worsening. There is daily SPG oversight to patient care and associated costs, including but not limited to, unnecessary care and services (medications, transportation, labs, diagnostic testing, durable medical equipment, etc.). SPG also works to identify high-risk patients and incorporates necessary resources to assure that the member is managed according to their risk(s). SPG is available 24/7 to assure that the patient has access to care at all times. This access to care and enhanced level of service prevents unnecessary admissions and readmissions to the hospital, allows for a shorter length of stay in acute and sub-acute settings and increases patient satisfaction.
