As the Discharge to Assess (D2A) Service Manager, I lead the strategic commissioning, operational delivery, and continuous improvement of pathways that support safe, timely, and person‑centred hospital discharge. My role sits at the intersection of health and social care, bringing together system partners to ensure that people receive the right support, in the right place, at the right time.I oversee the full D2A operating model, from demand and capacity planning to governance, performance oversight, and risk management. This includes translating complex modelling into clear, actionable insights that drive decision‑making across the system. I design and implement multi‑workstream action plans, ensuring each element is properly scoped, resourced, and aligned to strategic priorities such as flow, independence, and financial sustainability.A core part of my role is building strong partnerships across acute, community, social care, commissioning, and voluntary sector teams. I create the structures that enable shared accountability—governance frameworks, reporting cycles, escalation routes, and performance dashboards—so that operational teams and senior leaders have clarity, grip, and confidence in the system.Ultimately, my work ensures that D2A services are safe, effective, and resilient—supporting people to return home wherever possible, maximising independence, and enabling the wider system to function smoothly.
