Combination of case management & direct-care nursing utilized in this position. Frequently scheduling primary care, specialty, dental, & vision appointments for clients with priority population psychiatric disorders. Frequently coordinating with partnered pharmacy in order to assure prompt & appropriate delivery of medications for clients. Report to interdisciplinary meetings daily with additional team members in order to achieve the unified goal of meeting clients where they are. Direct care includes but is not limited to: assistance with Activities of Daily Living, Vital Sign collection, Phlebotomy, Intramuscular Injections.
