This role is about helping people across the system understand how to do PHM in a way that turns data into action and delivers real value to the people we serve. The team I lead provides analysis, advice and structured programmes of support to deliver this to people in various roles working in all sectors across the whole system. It also provides PHM resources and documents, supports in managing Information Governance requirements and develops networks across teams and sectors, and shares learning from local, national and international best practice.
