Engage new and existing patients with chemical dependency diagnosis.· Completes assessments with quality and clarity, using interdisciplinary expertise with relevant detailed information, i.e, age specific, cultural information, contact information.· Identifies homeless patients that are high risk.· Reviews/updates the assessments as new information is acquired, as circumstances and goals change, and as progress toward goal is made.· Participates in the identification, assessment, and intervention in cases of suspected abuse, neglect or exploitation of patients.· Utilization of appropriate documentation tools (i.e. flowsheets, SmartPhrase, etc.)· Conduct risk assessment to identify potential barriers to adherence to care.· Collaborates with the patient and health care team to develop and implement an individualized care plan and establish goals of the intervention.· Determine the type of services and resources that are available in order to address the established goals or desired outcomes.· Reviews and updates plan of care according to patient’s needs, Harris Health and Department protocol and/or regulatory agency requirements.· Consistently engages and utilizes internal and/or external community resources, as well as patient’s strengths, to meet patient needs.· Documents the care plan as well as: patient and/or family agreement with plan, education/teaching/resources provided to the patient/family, the list of facilities presented to the patient/family and that the choice of provider was given.· Appropriately adapts care plan and service/clinical methods to accommodate the unique physical, psychosocial, cultural, age specific and other developmental need of each patient served.

I hold a total of 8 years in management and am a licensed chemical dependency counselor working in the substance abuse field for 21 years. I graduate May 11, 2024 from Texas Southern University completing my Master\'s in Counseling and Education in Clinical Mental Health and intend to pursue an LPC.
