As the Medical Director of the Stony Brook PACC ( Post Acute Care Collaborative), my team and I work with 23 nursing homes and 7 home health agencies who are part of our PACC. Together, we develop mutual goals to improve quality, enhance the patient experience, and lower the cost of care. Through meetings with PACC Members, my team assist with development and implementation of standardized practices and processes across the PACC.
