Provide person-centered, culturally appropriate case management services tailored to each New American family’s unique needs- Conduct comprehensive needs assessments and develop individualized service plans in collaboration with clients- Connect clients to appropriate community resources, services, and benefits through coordination with Resettlement Network partners and other local agencies- Maintain strong, collaborative relationships with resettlement agencies, AmeriCorps members, and partner organizations to ensure consistent and effective service delivery- Monitor client progress toward increased self-sufficiency and make adjustments to service plans as needed- Accurately document all client interactions and services provided in accordance with program policies and contractual obligations- Prepare and submit timely monthly reports and maintain case files with up-to-date records and notes- Support families in navigating systems such as healthcare, education, employment, transportation, and housing- Uphold cultural competence by demonstrating sensitivity, respect, and responsiveness to the cultural and linguistic backgrounds of clients- Maintain client confidentiality and adhere to ethical standards in all interactions
