Conducts time-sensitive linkage to care interventions to coordinate medical, prevention, and other support services for people living with chronic diseases- Coordinates the collection and recording of data in accordance with the Ohio Department of Health Infectious Disease Reporting System- Communicates with medical and health facilities in order to expedite the administration of the target subprograms- Conducts orientation and training to infectious disease staff using technical education materials and references, demonstration, didactic teaching, and ongoing consultation- Prepares summaries and completes required paperwork; ensures the submission of required paperwork from assigned staff- Administers non-blinded HIV serosurvey questionnaires or other research protocols- Performs diagnostic tests and reviews labs in order to determine presence of infectious diseases- Assigned to serve as a member of a Disaster Recovery/Business Resumption team, Incident Response Team, or similar public health response team which may include the conduct of operations on a 24/7 basis at remote locations.
