At my supervised field placement role in Hospital Elder Life Program (HELP) at UPMC Shadyside, I support the prevention of hospital-acquired delirium in elderly patients, particularly those with dementia, by providing structured cognitive and social engagement interventions. I help create a calm and comfortable environment through companionship, therapeutic recreational activities, and regular interactions that promote orientation and cognitive function. I assist patients during mealtimes to ensure adequate nutrition and hydration, while monitoring for signs of distress or functional decline. I also communicate observations and concerns to the interdisciplinary clinical team, contributing to individualized care planning and helping ensure that each patient’s needs are met promptly and effectively. This role requires sensitivity, empathy, and a strong understanding of geriatric care practices in a hospital setting.
