Head of Investigations at Social Work England, bringing over 20 years of investigative expertise across criminal justice and healthcare settings.Previously, as Lead Serious Incident Investigator at South West Yorkshire Partnership NHS Foundation Trust, I drew on 17 years of safeguarding and domestic abuse investigation experience to strengthen patient safety and improve quality of care. I hold an MSc module in Healthcare Leadership and Management and am accredited in child protection, PIP2 investigation, and root cause analysis.My work has spanned systems analysis investigations, delivering training, and supporting colleagues while engaging with stakeholders at board level. I contributed to policy development, domestic homicide reviews, and safeguarding initiatives, including MARAC and MAPPA agendas. Passionate about achieving best evidence and driving equality, inclusion, and organisational change, I strive to embed systemic improvements that put people at the heart of everything we do.I played a key role in implementing the Patient Safety Incident Response Framework (PSIRF), guiding operational teams through a transformation in how the Trust responds to safety incidents—emphasising collaboration, systemic change, family engagement, and human factors. Recently, I completed the Home Office-accredited Domestic Homicide Review Chair course with AAFDA, adding further depth to my professional development.My philosophy is simple: quality improvement starts with systems and ergonomics, and people must remain at the centre of professional practice.As a South Asian working mum of two primary school-aged children, I champion flexible working and challenge the notion that parenthood and career success are mutually exclusive.
